Life After Top Surgery: Healing, Scars and the Power of Gender Affirmation

Undergoing top surgery is more than a physical procedure. It’s a deeply affirming step toward aligning your body with your identity. For many transmen, transwomen and gender nonbinary individuals, this surgery brings a sense of emotional and physical peace and authenticity that can be life-changing. But what happens after the operating room? Here’s a closer look at what to expect when it comes to healing, scar care and the profound emotional impact of life after top surgery.

The Healing Journey
Recovery after female-to-male (FTM), female-to-nonbinary (FTN) or male-to-female (MTF) top surgery initially is an ongoing process that requires patience, care and compliance with guidelines and instructions. Discomfort is relatively mild. In the first few weeks, in particular, there are more restrictions on physical activities in order to help facilitate ideal healing. Though recommendations do vary, all restrictions are lifted after three months.
Appropriate nutrition, both before and after your surgery, is key to ideal healing. This includes a substantial amount of protein, preferably animal-based. This is not the time to diet or be on a semaglutide like Ozempic®.
Scar Care Tips
Scars are an inevitable part of surgery as they always result from incisions that are made in the skin. However, with expert surgical techniques, temporary activity restrictions and proper postoperative scar care, these can fade and become far less noticeable over time.
Dr. Turkeltaub will provide personalized aftercare instructions that should be closely followed. Some of the general recommendations include:
- Follow wound care instructions carefully to minimize complications and support proper healing.
- Use the recommended silicone gel or silicone tape and do so as explicitly instructed. Using off-brands, like some that can be purchased on Amazon, can jeopardize your long-term results.
- Protect healing skin from the sun since UV exposure can darken scars.
- Temporarily avoid or minimize various physical activities, as enumerated, in order to reduce the risk of wider and thicker scars.
Everybody heals differently. Genetics, skin type and aftercare all play a role. What matters most is that your scars tell a story of courage, authenticity and affirmation.
The Emotional Impact of Gender Affirmation
The physical changes from top surgery are only part of the transformation. Many patients share that the emotional benefits are even more profound. No longer needing to bind daily, being able to wear clothing comfortably and feeling at ease in social or intimate settings can be and are life-changing.
Living with a chest that finally aligns with your gender identity often brings increased confidence, reduced or resolved dysphoria and an overall sense of freedom. It’s not uncommon for patients to describe top surgery as the most affirming step they’ve ever taken toward living fully and authentically.
Choosing the Right Surgeon
When considering gender affirming top surgery, choosing a surgeon who understands the unique importance of this procedure is essential. Look for:
- A reputable plastic surgeon, board-certified by the American Board of Plastic Surgery, who has extensive experience in top surgery techniques.
- A sincere commitment to individualized care, recognizing that everybody and every journey is different.
- Respectful communication that prioritizes your identity, pronouns and personal goals.
The right surgeon is more than a skilled technician; they are a partner who supports your journey toward authenticity.
