What to Expect for the Female-to-Male (FTM) and Female-to-Nonbinary (FTN) Top Surgery Recovery Process

The immediate surgical results from female-to-male (FTM) and female-to-nonbinary gender affirming (FTN) top surgery are critically important for anyone undergoing the procedure. Surgical excellence in the performance of the procedure can result in superb long-term results — physically and emotionally.

However, that is not the entire story. The postoperative “recovery” process is also crucial in affecting the long-term outcome. If instructions and protocols are not followed in the postoperative period, great results can be compromised leading to long-term outcomes that are non-ideal, average or even worse.

Consequently, no matter what gender affirming top surgery procedure you will have, proper preparation and strict adherence to aftercare instructions and guidelines can help shape your final results. Understanding this process can also help you realize the best possible outcome given that there are other factors out of your control.

FTM/FTN Top Surgery Recovery Timeline

The following are general postoperative recovery timelines and are not written in stone. There is considerable variability that will largely depend on the treating plastic surgeon’s practices and preferences and each patient’s unique characteristics as well as compliance with guidelines and instructions.

Immediately Postoperative (First Few Days)

Following top surgery, a protective dressing is placed which will remain on for around two to three days. This will then be replaced with a supportive garment that will be worn day and night for around four weeks. Your (two) drains will be left in place.

Discomfort for most patients is relatively mild. Many take little to no pain medication. You are encouraged to walk around frequently and not just lie in bed or sit on a couch or chair. There should be no vigorous activities and limited upper extremity usage.

Weeks 1–2

Expect to feel quite decent after several days with little to no discomfort. Most drains are ready to be removed between 8 and 14 days. If you have had the double incision mastectomy with the free nipple graft, the protective dressing is removed around 12 to 14 days.

Depending on the procedure you had, how you feel and other factors, you may be able to engage in limited driving, work or schoolwork from home or a light desk job at work.

1 Month

Depending on the specific procedure, scar gel or silicone tape is started at three to four weeks and will be continued for around six months or more. This will help with scar healing, resolution and aesthetics.

The postoperative garment no longer needs to be worn. Physical activities can be increased though still are restricted. Most patients can return to a desk type job or light duty depending on work requirements and surgical procedure.

By one month, there is already a significant improvement in the appearance of the chest as compared to soon after surgery.

Months 2–3

There are restrictions on heavy lifting for around two months. Specifics do vary among plastic surgeons and can be affected by the procedure performed and each individual’s degree of healing. At three months, there are no more restrictions on any activities.

During this period of time, swelling generally is noted to decrease and chest definition improves.

Months 6–12+

Your long-term outcome should become fully visible as swelling subsides and scars mature with flattening and dissipation of redness. The final results can take up to a year to be fully visible. However, even over a period of several years, the scars can become even more inconspicuous.

So, a very effective way to reduce your risks and improve your odds of obtaining a more desirable result is to closely follow your plastic surgeon’s recommendations and guidelines.

Follow-up Appointments After FTM/FTN Top Surgery

Showing up for your postoperative appointments is essential in your overall care. Not showing up for office visits can adversely affect your long-term results.

The initial postoperative visit, usually at two to three days, involves the removal of the surgical dressing and placement of the postoperative garment. Subsequent visits may involve:

Skipping or missing follow-up appointments can lead to missing early signs that can result in less-than-desirable long-term results that otherwise could have been addressed effectively earlier.

When Can I Expect to See My Complete FTM/FTN Top Surgery Results?

Your “final” results may take a year or more as the swelling resolves and the redness dissipates. Even over a period of several years, the scars can become even more inconspicuous. This, of course, depends on individual factors.

Scars from the keyhole and concentric circle (periareolar) techniques can even be inconspicuous by six months while those associated with the double incision and gender affirming breast reduction techniques will take far longer to be less noticeable.
